Abstract

Erythema nodosum leprosum, or type 2 reaction, is a type III hypersensitivity reaction. It is characterized by eruptions of painful nodules that last a few days or weeks and is generally associated with systemic involvement. It rarely occurs with the formation of necrotic ulcerated nodules. This variety has been called necrotic erythema nodosum leprosum due the important vasculonecrotic process that occurs. The necrotic erythema nodosum leprosum is a rare condition, occurring in severe type 2 leprosy reactions associated with high bacillary indexes, mainly in cases of longstanding multibacillar leprosy. It presents with significant cutaneous and systemic involvement. Clinical diagnosis is not easy, the necrotic lesions and the presence of ulcers, can cause confusion with other diseases.
