An erythematous-brownish patch in the left gluteus, visible since perinatal period, was found in a 1 year old boy. He presented with a pseudo-Darier sign. Histologically, smooth muscle hyperplasia in the dermis was observed with normal epidermis. Immunohistchemical stains such as smooth muscle actin, desmin and H-caldesmon, revealed the smooth muscle origin and confirmed the congenital smooth muscle hamartoma diagnosis.
