Acquired perforating dermatosis with removal of collagen and elastic fibers associated with erythema annulare centrifugum in a man with metabolic comorbidity

Abstract

Perforating dermatoses constitute a group of diseases characterized by transepidermal removal of dermal material, manifested as papules with a central keratotic crater, and are mainly described in patients with advanced chronic kidney disease. Centrifugal erythema annulare is a skin pathology associated with multiple comorbidities. We present the case of a patient with metabolic comorbidity who presented with both skin pathologies simultaneously, confirmed by biopsy, and with improvement after reaching metabolic control.

Mixed transepidermal removal is rarely described in the literature, and according to its pathophysiology, it may be associated with recurrent trauma and a persistent inflammatory state of the skin. In addition, it is rare to find it associated with another skin pathology such as centrifugal erythema annulare; the presence of both entities could be due to poor control of the patient's comorbidities.
