Mesenchymal cells and their extracellular vesicles: an alternative proposal for immunoregulatory and regenerative management in patients with psoriasis

Psoriasis is a chronic inflammatory disease that affects 2 to 3% of the world's population. It is caused by an abnormal activation of the immune system, mainly by the IL23/Th17 and IL12/Th1 pathways, generating hyperproliferation of the keratinocytes, and a systemic inflammatory phenomenon. This pathology produces high morbidity and negatively impacting the quality of life of those who suffer from it. Various therapeutic strategies have been developed for the local and systemic management of the disease based on the immunosuppression of the activation process. The limitation of these treatments are the side effects and the reappearance of the disease once the treatment is suspended. In recent years, an innovative therapy that seeks to immunoregulate the process is the application of mesenchymal stem cells (MSCs) and/or extracellular vesicles obtained from MSCs, these therapies have demonstrated good results through their action in regeneration, differentiation and immunomodulation, a narrative of the updated literature is made to focus the vision of this promising therapy
