Acute generalized exanthematous pustulosis in a young patient

Abstract

Acute generalized exanthematous pustulosis (AGEP) is a rare dermatological pathology, characterized by the sudden appearance of skin lesions in the form of multiple, punctate, sterile pustules, on an edematous erythematous background. It has generally been associated with multiple drugs, including exposure to certain antibiotics. We present the case of a 34-year-old male patient who was admitted to the emergency department due to the appearance of pustular lesions of two weeks' duration, pruritic, some ulcerated, with signs of superinfection, which involved the four extremities, genitals, anterior and posterior trunk, respecting mucous membranes. Admission tests showed a leukemoid reaction with a leukocyte count of 83,000 cells/mL, predominance of neutrophils (75,250 cells/mL) with eosinophilia of 2,520 cells/mL and elevated C-reactive protein (CRP) (29.2 mg/dL). In the skin biopsy, they reported changes suggestive of PEGA, with improvement and resolution of the inflammatory skin manifestations after treatment with systemic steroids.
