Abstract

Among cutaneous melanomas, lentigo acral melanoma is one of the rarest subtypes, accounting for approximately 2-8% of cases in adults. In the pediatric population, its clinical manifestation tends to be more aggressive and can present atypically, making early diagnosis challenging. Due to the rarity of this type of melanoma in children, careful attention is required from the initial evaluation, with incisional or excisional biopsy being the cornerstone for diagnosis. This report presents the case of a 3-year-old male preschooler with a presumptive positive Hutchinson's sign for subungual melanoma on the left hand's hallux, where the biopsy plays a crucial role in establishing a definitive diagnosis.
