Verrucous and exudative plaque on the scrotum

Extramammary Paget's disease is a rare malignant epithelial neoplasm, frequently found in body areas rich in apocrine glands. The external genitalia are often affected, more so in women. Treatment depends on the stage of the disease, and wide local resections can be performed.
We present the case of a patient with a lesion on his scrotum who was managed as having a superficial fungal infection for 2 years without showing any improvement. Clinical suspicion, biopsy, and immunohistochemistry helped us confirm the diagnosis.
