How to manage the non-ocular complications from hyaluronic acid? Proposal for a therapeutic algorithm

Background: In the last years, there has been a documented increase in the frequency of injectable aesthetic procedures, consequently leading to a rise in complication rates.
Among these, vascular occlusion is the most feared due to its high morbidity for the patient and the stress-induced confusion it may cause in the injecting doctor. Due to this, it is imperative to establish a clear and simple step-by-step approach that injecting physicians can quickly access. This would serve as a tool to calm and guide them in the therapeutic approach swiftly, effectively, and efficiently.
Methods: A literature search was conducted, based on this review and the experience of the injectors, objectives were determined to optimize the patient's attention experiencing vascular events due to hyaluronic acid. An algorithm was developed to incorporate the most validated appropriate clinical practices in the real-life setting.
Results: Based on injectors recommendation with over ten years of experience and on literature reviews and evidence, a simple and clear therapeutic algorithm was created. It clarifies a practical step-by-step approach to be followed in the case of a vascular adverse event due to hyaluronic acid.
Conclusion: Although there are related algorithms reported previously, we offer a guided step by step, from the moment of the patient’s initial assessment to the final follow-up when it is decided to discharge the patient. With information that can be forgotten due to stress produced by the adverse event, reminders and basic information such as what the emergency kit should have.
