Abstract

Background: Cutaneous Mastocytosis (CM) is a rare entity in pediatrics, it is the most common clinical presentation of mastocytosis and is characterized by a variable clinical presentation, in which subtypes have been identified such as: maculopapular, bullous, nodular, diffuse cutaneous, mastocytoma, among others.

Case summary: We discuss the case of a 4-month-old patient who was admitted to the emergency department due to fever and generalized bullous lesions. An initial diagnostic approach to the febrile syndrome due to Staphylococcal Scalded Skin Syndrome (SSSS) was carried out, with the subsequent performance of a skin biopsy, which identified superficial dermal and perivascular infiltrate of predominantly mononuclear with interstitial mast cells, which was identified as cutaneous bullous mastocytosis and comprehensive management was initiated.

Conclusions: Since cutaneous mastocytosis is an uncommon entity in pediatrics, for that reason it is extremely important to obtain an early and accurate diagnosis, since late establishment of appropriate treatment could lead to the development of systemic forms that could put the patient's life at risk. Comprehensive treatment will depend on the extent of the lesions and the involvement of other systems, varying from pharmacological management to surgical procedures for debridement of necrotic tissue.
