Herpes zoster ophthalmicus in a pediatric patient: case report

Introduction: Herpes zoster is an infection resulting from the reactivation of the varicella zoster virus, characterized by the appearance of painful vesicles that follow the territory of a dermatome, produced years after the first contact with the virus. The main risk factors for this disease are: advanced age and cellular immune dysfunction. The appearance of this condition in immunocompetent children without a personal history of chickenpox is rare. Maternal chickenpox is one of the main risk factors for childhood herpes zoster during the first years of life.
Clinical case: A case of a 5-year-old female preschool patient is presented, who presents painful vesicles in the left frontal and periocular region. Reason why she is taken to the pediatric emergency of the Ciudad Hospitalaria “Dr. Enrique Tejera”, where they evaluate and enter, requesting evaluation by the Dermatology and Ophthalmology Service. In view of clinical and paraclinical findings, the diagnosis of ophthalmic herpes zoster was established, and treatment was instituted with intravenous Acyclovir (30 mg/kg/day) and topical for 10 days, and antibiotic therapy due to periorbital cellulitis, presenting subsequent clinical improvement.
Conclusion: Therefore, a high clinical suspicion is required to make the diagnosis and establish adequate treatment and follow-up to avoid complications or recurrences.
