Juvenile pemphigus vulgaris. About a case

Pemphigus vulgaris is a blistering disease of autoimmune origin, which affects skin and mucous membranes, and is mediated by autoantigen-directed anti-IgG autoantibodies, Dsg 1 and 3 present in keratinocytes. Causing loss of cell adhesion, just above the basal layer, forming intraepidermal blisters. The case of a 14-year-old adolescent with no significant history is described, who presented bilateral and symmetrical generalized dermatosis on the oral mucosa, characterized by flaccid blisters, some exulcerated and hematocysts on the surface, positive Nikolsky sign, painful for 1 month of evolution. A skin biopsy was performed, which he reported; suprabasal intraepidemic acantholytic blister and mild superficial perivascular lymphocytic inflammatory infiltrate in the dermis. Treatment was started with Wonder's magistral solution, topical and systemic steroids and immunosuppressants with a good response. A case is reported because it is an infrequent disease in adolescence, I feel its peak of appearance between the fourth and sixth decade of life.
