Scalp metastasis of a thyroid follicular carcinoma: case report

Cutaneous metastases from thyroid cancer are rare, usually occurring in the context of disseminated disease. We present the case of a 53-year-old patient, with a history of left partial thyroidectomy 22 years ago for a follicular adenoma, who debuted with metastasis in the parieto-occipital region in which thyroid tissue was identified, immunohistochemistry was positive for thyroglobulin, CK7, and TTF1. In the thyroid ultrasound, post-surgical changes due to left hemithyroidectomy and of the isthmus are identified, with a 28mm hypoechoic nodule in the right lobe that extends to the submandibular region, with peripheral vascularization, categorized as TIRADS 4A, for which a histopathological study is performed with BACAF BETHESDA IV/VI. The patient underwent a total thyroidectomy, with histopathological study of the thyroid gland reporting a follicular thyroid cancer, indicating a recurrence.The patient died 2 months after the cutaneous metastasis.
