Lesions of leprosy reverse reaction mimicking keloids

Abstract

Leprosy is an infectious disease, curable, and capable of generating deformity and disability, if not diagnosed and treated early. About 20 to 50 % of patients present reactions during the course of the disease, more frequent after treatment has been initiated. Type 1 reaction, or leprosy reverse reaction, occurs mainly in patients with dimorphous-tuberculoid, dimorphous and dimorphous-lepromatous leprosy. We report the case of a 52-year-old male patient, who came to the leprosy outpatient clinic at ASA-FIOCRUZ in Rio de Janeiro (Brazil), presenting exuberant and painful skin lesions, associated with myalgias, systemic symptoms and pa-restesias in the extremities. After the anamnesis and physical examination, it was possible to identify these lesions as an atypical clinical form of type 1 reaction, which was later corroborated with the help of paraclinical studies and the resolution of the clinical picture in the treatment established for leprosy and reactive state.
